Appointments for all New Referrals (Consultations, Assessments and Management)

Dr Nazrin Lee

As a standard practice, new referrals for assessment and management

will require four appointments, which usually comprise of:

A Consultation and Assessment of Patient with Parents

A Consultation and Assessment with Parent(s) only

A Consultation and Assessment with Individual Patient

A Feedback and Management Consultation.

FOR NEW REFERRALS

Download and complete Dr Lee's Patient Registration Form (and along with your GP referrals),

Please then, email it to: admin@themindhealingplace.com.au

If your referral has been accepted, Dr Lee's Practice Info PDF will be sent out to you.
